Discuss your decision with your family physician. Your primary care physician support is important.
Contact your insurance provider regarding bariatric benefits
Schedule initial surgical evaluation with our office. Call (405) 691-3380.
Fill out the surgical application located on this site under "Patient Information" and via the "How to Get Started" link. (Download patient information and health information documents, fill in prior to appointment and provide information to our office)
Sign the release of information request form located on this site. (Download this form and and fax/mail to our office so we can acquire your medical records from your primary care physician).
